On July 8, 2015, the covered entity (CE), Minneapolis Clinic of Neurology, Ltd., discovered that a laptop computer was missing from one of its clinics. The jade affected approximately 1,450 individuals and the types of protected health information (PHI) involved in the cut included patients' names and addresses. The ce provided hack notification to HHS, affected individuals and the media. following the hack, the CE sanctioned the involved employee with a written warning, distributed its computer network and net access policy to all employees, and retrained all employees ahead of its annual training. The CE also implemented policies and procedures contained in a new HIPAA secrecy and security Handbook, increased technical and security safeguards on its mobile electronic devices, and updated the security on its virtual private network software.
